If you go to this site and scroll down, you can see an image one side of the north gate of Milecastle 42 on Hadrian's Wall showing the pivot hole for one leaf of the double gate. This clearly swings inwards into the recess. It is true that milecastles were fortlets, rather than fortresses, but I think that this establishes the principle.
