With lost wax/investment (plaster) mold making in modern times with vacuum or high pressure assistance, one can cast a hole for a hinge or other purpose. But using only gravity, I don't think it could be done repeatably or effectively.

Some other metal casting voice may have a better answer.

Well here is the latest progress on the pugio. The handle brass is only just started and these are not the rivets I will be using for final fixing. I intend to make some as close to the originals as I can make out from photographs. Still some work to do on the blade as well.

Sulla Felix ask me to buy a pugio blade made by Pastor. I haven't found any post service who garant the blade arrives sure to him, so i can't send the blade. Now he makes that beautiful blade, so finally, he have a blade and can make the reconstruction.

No worries Cesar - I understnad how difficult it is to get things posted, particularly a blade!! I still hope that one day we can arrange for collection of the blade (somehow, somewhere). My work is nowhere near as good as Pastor's work!
